Flagstaff, Ariz. – A baby died after a crash involving an off-highway vehicle (OHV) south of Flagstaff.
Just before 11am on Sunday, the Coconino County Sheriff’s Office responded to a crash fall involving multiple casualties on Forest Service Road 535 and State Route 89A, near the top of Oak Creek Canyon.
When deputies arrived at the scene, they discovered that the OHV had crashed and rolled.
The vehicle was operated by 37-year-old Miguel A. Pacheco. The passengers were identified as babies aged 17, 7 and 19 months old.
The older children were treated for injuries on-site.
He was 19 months old and was killed when he was kicked out of the vehicle during a collision. CCSO says the baby was not in the child’s safety seats or in the proper child control.
At this time, the relationship between the driver and the child is unknown.
After the investigation, Pacheco was arrested and placed in jail on charges of possible manslaughter charges, dangerous assault, felony risk, possession/use of drug drugs, DUI and reckless driving.
The investigation is still underway.
