News Flash • Community Health Needs Assessment Published

Coconino County Health and Human Services (CCHHS) released the 2024 Coconino County Community Health Needs Assessment (CHNA) on March 5, 2024.

The CHNA is a comprehensive assessment conducted every three years to deeply understand community health conditions, risk factors, causes and concerns of poverty, and available resources within Coconino County. The main purpose of this assessment is to understand the health of the community. Identify health disparities. Establish a path to improvement. and reduce barriers.

CCHHS is a community action agency tasked by the federal government with investigating poverty in our communities. As part of the CHNA process, CCHHS conducted a survey to learn about the experiences of community members. Partner agencies were asked to distribute the survey in order to provide the survey broadly. In addition, a survey of service providers was conducted to hear directly from communities and those who work with clients.

CCHHS collected and analyzed the data, and the Coconino County Community Action Advisory Board (CAAB) identified the most pressing community needs and assets based on available data. The top priorities identified by CAAB are:

  • housing
  • food and nutrition security
  • behavioral health
  • Transportation facilities

CCHHS will continue to work with communities to conduct focus groups across the county to explore top health priorities and identify strategies and approaches. This next phase of work will result in an updated Community Health Improvement Plan (CHIP), extending the existing 2022 to 2026 CHIP and specifying health priorities into categories such as: and take further action.
