The crash occurred Saturday afternoon just west of Prescott Regional Airport near Williamson Valley Road and Levy Lane.
PRESSCOTT, Ariz. — The Yabapai County Sheriff’s Office said her father and two children died after a head-on collision in Prescott on Saturday.
The Central Arizona Fire Department and Medical Department told 12News that a crash occurred at around 3:01pm on Saturday at Williamson Valley Road near Levy Lane. The area is just west of Prescott Regional Airport and just east of the granite mountains.

YCSO said deputies discovered that their father and one child had died at the scene of the crash. The second child died while being taken to the hospital. Three adults were airlifted to hospital to treat injuries. The third child is in a critical state at a Phoenix hospital.
YCSO said the crash occurred when a man identified as Chino Valley’s Jesse Forman drove Williamson Valley Road as a Toyota 4 Runner and collided head-on with a Jeep SUV across the center line.
Forman was driving on a four-runner with his three children and their mother, but the woman was identified as being in her 60s, and her son, who was only identified as an adult, was in the Jeep.
The woman and her son were both airlifted to a hospital in Flagstaff, and YCSO says they are seriously injured.
YCSO did not say how serious the mother’s injuries were.
“The Sheriff’s Office is expanding its deepest sympathy for the children and families involved in this horrifying crash,” Yavapai County Sheriff David Rhodes said in a press release. “I would also like to recognize the tireless efforts of many first responders who quickly rushed to help all involved.”
YCSO said lawmakers are making extra efforts to enforce traffic on Williamson Valley Road due to “recent public complaints.” The sheriff’s office says it will continue to patrol the area where the crash occurred and other parts of the county where reckless driving is in question.
